I also give permission for medical/hospital/ambulance services to be called if I am unable to be contacted in the event of an emergency, illness or accident concerning my child/children listed above and agree to pay such costs that may be incurred. 

I also give permission for my child to be photographed videoed with the understanding that these will not be displayed outside the activities of Southside Church.
